Intellisense not working after version 1.58

Beginning with version 1.59.17466, Intellisense no longer works in Visual Studio Code. Help popups when hovering over variables, functions, etc. no longer appear and the extended context-menu items are missing.
With 1.58.17233:

Beginning with 1.59.17466:

All later versions to date behave this way.

It may be incorrectly installed or initialized extension. The 1.59 works on all our test machines. The following is from 1.59.17674:

Thank you. Problem appears to be outdated VS Code version preventing PHP Language Server from starting with 1.59 installed.
